Description The Discharge Coordinator provides discharge
services through a variety of support functions for the patients,
their parents/guardians, family therapist and medical providers;
coordinates and manages all written and oral communication
inter/intra-departmentally. Providence caregivers are not simply

Discharge Planner Full Time Day-Shift Required qualifications: -
Bachelor's Degree in Mental Health/Social Services Or 4 years of
Equivalent Experience when not having a bachelor’s degree. -
Coursework/Training and Residential Treatment, Acute Care Inpatient
or Behavioral Schools. Preferred qualifications: - 2 years of
experience in an inpatient mental health setting. - 2 years of
Adult Unit: Experience with adult population preferably in an
inpatient setting. - Child and Adolescent Unit: Experience with
child/adolescent population preferably in an inpatient setting. -
Senior Unit: Experience with geriatric population preferably in an
