Description The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. Required Qualifications: - Bachelor's Degree In PHARMACY from an accredited School of Pharmacy or, - PharmD - In PHARMACY from an accredited School of Pharmacy. - Coursework/Training - Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent educ/experience. - Oregon Pharmacist License upon hire. - Certification from Board of Pharmacy Specialties within 2 years of hire. - TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement. Preferred Qualifications: - Board of Pharmacy Preceptor license when eligible and where applicable upon hire. Why Join Providence ? The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
