Description The Clinical Pharmacist Specialist will collaborate
with physicians and staff to provide coordinated, comprehensive,

Bachelor's Degree In PHARMACY from an accredited School of Pharmacy
or, - PharmD - In PHARMACY from an accredited School of Pharmacy. -
Coursework/Training - Postgraduate Year One (PGY-1) AND Year Two
(PGY-2) Pharmacy Residency or equivalent experience or equivalent
educ/experience. - Oregon Pharmacist License upon hire. -
Certification from Board of Pharmacy Specialties within 2 years of
hire. - TWO years of Senior Ambulatory Clinical Pharmacist
experience within relevant practice setting - OR - FIVE years of
pharmacy practice experience with at least 2 years of experience in
relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
Preferred Qualifications: - Board of Pharmacy Preceptor license

for this position. The successful candidate's placement on the wage
range for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
