Signify Health, a pioneer in technology-driven healthcare and value-based payment programs, is seeking a dedicated Nurse Practitioner for In-home Visits to join our dynamic team. This role is centered around providing direct, in-home care to patients, guided by a streamlined platform that utilizes advanced analytics and technology. The ideal candidate will proactively address patient healthcare needs, promoting preventative care and managing treatment plans in a home setting, thereby significantly contributing to patient outcomes and satisfaction. Conduct comprehensive assessments of patient health needs during home visits. Develop and manage effective, individualized care plans. Educate patients and their families on preventive care techniques. Regularly report patient status and progress to the assigned medical team. Collaborate with other healthcare professionals to streamline continuity of care. Use the company’s technology to update patient records and coordinate with healthcare providers. Strong clinical skills with a focus on elderly and chronic care management. Excellent communication and interpersonal skills, capable of explaining complicated health concepts to diverse populations. Proficiency in the use of healthcare technology and electronic health records. Ability to work independently with minimal supervision. Malpractice coverage Valid Nurse Practitioner license. Board Certification from the American Nurses Credentialing Center (ANCC) or equivalent. CPR and Basic Life Support (BLS) certifications.
