Job Description:
Legacy Health, a renowned non-profit health system operating
across Oregon and Washington, is seeking a compassionate and
dedicated Project Nurture Peer Mentor to join our Community
Services team. This crucial role supports pregnant individuals and
parents with substance use disorders, guiding them through the
continuum of care, and providing emotional support and advocacy
within a structured health setting. The Project Nurture Peer Mentor
will use personal experiences in conjunction with formal training
to offer support and insight, fostering a proactive and
recovery-oriented environment. Provide one-on-one mentorship and
support to mothers and pregnant individuals recovering from
substance use. Facilitate access to healthcare services, including
scheduling and accompanying clients to appointments. Assist in the
development and implementation of personal recovery plans. Offer
emotional support and motivation to encourage clients’ adherence to
their health and recovery goals. Maintain confidentiality and trust
with sensitive information. Document and track the progress of
clients, reporting to healthcare professionals as necessary. Strong
interpersonal and communication skills Ability to relate personal
recovery experience effectively and constructively Knowledge of
substance abuse disorders and the recovery process Crisis
intervention skills Medical Insurance Dental Insurance Vision

certification First Aid and CPR certifications
