Job Description:
Legacy Health is actively seeking a dedicated Clinical Social
Worker II LCSW to join our compassionate team, focusing on
providing top-tier social services at our healthcare facilities
across Oregon and Washington. This position offers the opportunity
to work in a dynamic environment where patient care is centered on
a holistic approach, integrating physical and mental health to
create comprehensive treatment plans. The ideal candidate will be a
licensed clinical social worker with a strong background in mental
health and a passionate commitment to community wellness. Conduct
psychological assessments and provide psychotherapy to individuals,
families, and groups. Develop and implement treatment plans based
on clinical experience and knowledge. Collaborate with healthcare
providers to ensure that patient care considers both mental and
physical health aspects. Provide crisis intervention and support in
emergency situations. Advocate for patients and their families to
connect them with necessary community resources. Maintain
comprehensive documentation of patient interactions and
interventions. Educate patients and their families about mental
health and coping mechanisms. Proficiency in clinical assessment
and diagnosis Strong interpersonal and communication skills
Expertise in evidence-based therapeutic interventions for a diverse
patient population Ability to handle high-stress situations Case
management experience Knowledge of healthcare and patient privacy
regulations Proficiency with electronic health record systems
