ASAP About the Position Join Our Team as a Family Practice Nurse
Practitioner – Make a Difference in Patient Care Are you a
dedicated Nurse Practitioner seeking a rewarding opportunity to
provide high-quality healthcare to diverse populations? We are
looking for a compassionate and skilled Family Practice Nurse
Practitioner to join our dynamic team in Oregon's Lane, Multnomah,
or Washington counties. This role offers a unique blend of
telehealth and in-home visits, empowering you to deliver
comprehensive care while enjoying flexible scheduling and excellent
support. Role Overview As a Family Practice Nurse Practitioner, you
will primarily conduct telehealth consultations with some in-home
visits, focusing on pediatrics and adult populations. Expect to see
approximately 12-14 patients daily via telehealth or 6-7 in person,
with a balanced mix depending on scheduling. Your responsibilities
include performing annual risk assessments and managing chronic
condition reviews—an assessment-only role with no treatment or
prescribing. You will escalate urgent findings to primary care
providers, supporting continuity of care. The position offers
full-time or part-time options, with schedules released 2-3 weeks
in advance. Mileage reimbursement and flexible hours make this an

Skills Valid Oregon Medical License and active Medicaid
credentialing (must be highlighted in application) Proven
experience in family practice, pediatrics, or adult primary care
Strong communication and interpersonal skills Ability to perform
telehealth consultations confidently and professionally Comfortable
and capable of conducting in-home visits when scheduled Proficiency
in risk assessments and chronic condition reviews Ability to work
independently and manage a flexible schedule Nice to Have Skills
Experience with home health services and in-home patient visits
Knowledge of electronic health record systems such as Salesforce
Bilingual abilities or additional language skills Experience
working in telehealth environments or remote patient management
Preferred Education and Experience Master’s Degree in Nursing (MSN
or DNP) from an accredited program Minimum of 2 years’ experience
in family practice or primary care settings Current Oregon Nurse
Practitioner license and valid DEA registration Other Requirements
Enrolled and credentialed in Medicard Ability to travel within
designated counties; mileage reimbursed at IRS standard rate
Willingness to commit to a 3-month contract with possible extension
Possession of reliable transportation to conduct home visits This
