qualifications will not be considered. A contract opportunity is
available for a Clinical Sterile Compounding Pharmacist starting
May 1st, located in Redmond, WA. This position offers a Monday to
Friday schedule from 8:30 AM to 5:00 PM over a 3-month duration.
This role requires a WA Pharmacist license that is clear and
active. Candidates must have a minimum of two years recent
experience in an acute-care setting or at least one year recent
experience in home infusion therapy. A strong knowledge base in
intravenous admixture services and intravenous therapy is
essential. The ideal pharmacist will demonstrate the ability to
assume responsibility for evaluating and monitoring drug therapy
tailored to the complex needs of home care patients, collaborating
closely with physicians, nurses, and other healthcare
professionals. Effective teamwork and communication skills are
critical for delivering safe and effective drug therapy. Key
Responsibilities: Evaluate patient eligibility for home care
services and assess the appropriateness of prescribed therapies.
Develop and maintain patient-specific pharmacy care plans,
including establishing therapy goals and medication management.
Analyze laboratory results and perform pharmacokinetic dosing to
optimize medication regimens. Provide clinical dosing
recommendations and document interventions accurately in patient
records. Maintain precise patient medication profiles and conduct
prescription order entry to ensure proper medication preparation,
compatibility, and storage. Dispense medications and related
supplies ensuring all required documentation, authorizations, and
billing signatures are obtained. Collaborate with multidisciplinary
teams to promote safe, effective, and cost-efficient patient care.
