Typical caseloads include adult and geriatric patients recovering from surgery, managing chronic conditions, or transitioning from inpatient facilities to home.Your typical day will involve traveling between patient homes across assigned territories in the Portland area, coordinating closely with a supportive interdisciplinary team that may include nursing, occupational therapy, speech therapy, and social work. You will complete initial assessments, revisit patients for ongoing therapy, and collaborate on care plans designed to reduce falls, prevent rehospitalization, and optimize functional outcomes. Visit volumes and patient assignments are structured to promote quality care and allow time for thorough documentation.You will use a modern electronic medical record system and mobile tools to document visits, communicate with colleagues, and manage your daily schedule.
