(BSCM) is a locally-owned company specializing in geriatric care
management. The mission of BSCM is to help plan and fulfill the
needs of aging seniors, while ensuring they achieve the highest
quality of life by accessing trusted resources throughout Bend and
Central Oregon. BSCM works closely with each of its clients to
assess their needs, develop a care plan, coordinate services, and
monitor care; BSCM also serves as an advocate for seniors and acts
as a liaison between families and service providers. Role
Description Bend Senior Care Management (BSCM) is looking for
thoughtful individuals who are interested in a part-time Care
Manager position. The Geriatric Care Manager will be responsible
for assessing the needs of clients, creating and implementing
personalized care plans, and coordinating with healthcare
providers, family members, and other care professionals.
Responsibilities also include providing ongoing case management,
monitoring client progress, and ensuring the highest quality of
care and support for older adults to enhance their overall
well-being and quality of life. Qualifications Knowledge of Care
Planning and Care Management practices Experience in Gerontology
and Geriatrics, with a focus on elder care A strong desire to serve
older adults and their families through the aging process Excellent
communication, organizational, time management and problem-solving
abilities Compassionate, patient, and dedicated to improving the
quality of life for older adults Bachelor's degree in Social Work,
Sociology, Gerontology Valid driver's license and reliable
transportation for client visits
